How long does it take to get approved?

Responses typically arrive within 12 business hours after completing intake forms.

What information is needed for an LMN?

You’ll need to provide your name, contact info, birth date, symptom details, medical history, and identity verification. This information remains confidential and shared only with our healthcare providers.

How long is an LMN valid?

LMNs are valid for 12 months from the date of issuance. Note that LMNs cannot be used retroactively.

How do I submit my LMN to my HSA/FSA provider?

Submit your LMN and itemized receipt by logging into your HSA/FSA account, locating the Reimbursement or Claims section, and uploading the documents.

Note: Some HSA/FSA administrators may deny reimbursement; it’s advisable to confirm eligibility with your administrator beforehand.

Why might my LMN request be denied?
